Usually because the visible damp is not where the water enters. Water tracks along beams, behind renders and down cavities before it appears. Sealing the stain rather than the entry point is the most common reason a repair does not hold.
Sometimes, and crack injection and internal tanking both have their place. But water pushing from outside is best stopped outside wherever access allows, because internal work holds the water in the structure rather than off it.
